How it works for: Parent or guardian

1. Start from your practice's page

   Use the link the practice gave you, then create an account.
2. Request an evaluation

   Tell the office about your child. This opens a chart under your login.
3. Send records

   Upload school records and past reports under Documents.
4. Follow the steps

   My care shows which stage the evaluation is in. You are told when anything changes.
5. Go to the visits

   The interview, testing and feedback visits appear on My care.
6. Get the report

   When the clinician shares it, the report is in Documents.
